You can download a blank assessment form for your own use. Select the type of file best suited to your needs:

Microsoft word document . This document will open in most word processing programs (for both Windows and Mac computers) but if you have problems opening the file then download the RTF file below. (37 KB in size)

Once you have downloaded the assessment form you can use it electronically or print out a copy to record your details. Below are instruction to assist you in making a word document for each of the 'Units of Competency' you want to be assessed for. This allows you to build your own assessment portfolio for the qualification you are interested in.

How to create your own assessment form for your Units of Competency

You may want to simply print out a blank assessment form and hand write the Unit Code and Unit Name and the elements, but if you would like to have an electronic copy please follow the instructions below.

Firstly, may wish to print this page for easy reference while you follow the instructions. Use the print button on your browser.

  1. Go to the Unit of Competence you wish to be assessed for. These are located clicking on the qualifications e.g. Certificate IV
  2. Make sure you have opened in your word processing progam the 'blank assessment form' you downloaded.
  3. From the web page which contains your Unit of Competency select (highlight) and copy the 'Unit of Competency Code' located in the top left hand corner
  4. Go to the blank assessment form in your word processing program and 'paste' into the area for the 'Unit of Competency Code'
  5. Repeat this for the 'Unit of Competency Name'
  6. Next, go back to the web site and select (highlight) and copy the first 'Element' from the webpage then go back to the word document and paste into the cell of the table under the heading 'Elements' in the left hand column.
  7. Continue to do this for all the 'Elements' making sure you add each element to an individual cell below the previous one.
  8. When you have completed copying all the elements to the blank assessment form, type in your details at the top of the page.
  9. You can delete any empty rows from the table using your word processing program.
  10. You now want to save your document. Go to 'Save As' in you word processing program and save your file with a new name. If you name your file by using the Unit of Competency code (e.g.CHCADMIN1A.doc) you will find your file easily.
  11. You can now print out your assessment form and add it to your evidence portfolio or use it with your assessor.
  12. If you print a copy of the 'Unit of Competency' from the website you will then have a hard copy of all the elements, performance criteria, range of variables and evidence guide information for each of the Units of Competency you want to be assessed for.
